While interning in grad school, Michelle’s career path clicked into focus. Many people with her background go into commercial fitness, being a personal trainer, or become a coach. Her realization was that her skills could be used to help people be healthy, strong, and safe, and to be of service.

Happier & Healthier
Vitalize 360 supports individuals as they progress to living their “best lives.” Meetings give residents opportunities to share what is most important, set goals and refocus on living more intentionally. The active engagement of a staff and residents as a team is vital to program success. The team is a critical resource, plays an advocacy role and collectively supports individuals to achieve their goals.
Vitality Plan & Evidence Based Assessment
Individuals create a Vitality Plan, a personalized well-being roadmap that guides the achievement of self-identified goals. The Vitality Plan may support existing activities or inspire new achievements. Information about the person is gathered using two evidence-based assessment tools — the Lifestyle Survey and the Health and Social Check-up.
The Lifestyle Survey is self-administered and captures the person’s preferences and satisfaction with current activities. Information compiled from these two tools are documented and used for tracking, benchmarking, and resident improvement.
The Health and Social Checkup includes items in four key areas:
- Community and social life
- Cognition and mental health
- Getting around in everyday life
Wellness & Health
Vitalize 360 combines an innovative, award-winning, person-directed approach to wellness coaching and the power of information derived from a scientifically-grounded assessment system. The program is a joint venture between Hebrew SeniorLife, a Harvard Medical School Affiliate, and Kendal. It engages, challenges and inspires older adults to live full, healthy, vibrant lives.
Quality of Life
Vitalize 360 improves health, quality of life, mood, loneliness, perception of health status, and drives social interaction. It helps a community move from “what’s the matter” to “what matters most.” It increases the quality and depth of engagement between staff and residents, a requirement for really knowing each other. Staff records information and all the touch points along the way to help individuals live vibrantly and independently longer.
